Payment / Co-pay
We make every effort to keep your health care costs minimal. If you have patient responsibility (i.e. co-pays and deductibles) payment is expected upon registration, please plan accordingly. Failure to meet your responsibility will result in the rescheduling of your appointment. In cases of financial difficulty, we will be happy to work out a reasonable payment schedule. This must be done prior to your visit. We accept cash, checks, MC, Visa, Discover and Debit cards.
Any returned check will be assessed a charge for bank fees. All accounts over 30 days old will be subject to a $25.00 late fee.
Insurance
Please bring your insurance card(s) so that we may copy them. We do participate with most major insurance plans, but you are responsible for knowing if we participate with your plan. Please understand that your agreement is between you and your insurance company. You are responsible for the payment of your bill regardless of the status of your insurance company. There will be a charge for insurance, disability, FMLA, chart copies, and/or any miscellaneous form completions.
