New Patients
We appreciate the opportunity to serve your urologic health care needs.
We see patients by appointment only to ensure that sufficient time is allotted for each patient’s care. We believe in the value of your time, and will do our best to keep you from waiting. We would appreciate a 24-hour notice if you find it necessary to cancel your appointment. Please note that any “NO SHOW” appointments will not be rescheduled and will be charged $25.00.

Insurance
Please bring your insurance card(s) so that we may copy them. We do participate with most major insurance plans, but you are responsible for knowing if we participate with your plan. Please understand that your agreement is between you and your insurance company. You are responsible for the payment of your bill regardless of the status of your insurance company. There will be a charge for insurance, disability, FMLA, chart copies, and/or any miscellaneous form completions.
Copays & Payments
We accept cash, checks, MC, VISA, Discover, and Debit cards. Any returned check will be assessed a charge for bank fees. All accounts over 30 days old will be subject to a $25.00 late fee.
Referrals
Managed care patients are responsible to obtain a referral for all visits at our office, if necessary. Please contact your primary care physician as soon as possible to assure your referral has been completed. If you come to the office without a referral, you may be asked to reschedule.
